Bad idea. It's useless against bots, ineffective against paid spammers and very annoying to everyone else.

I agree although I would also add posting a song and a message in the chat before being able to participate in the forum.
It wouldn't hurt to change the "captcha" question from time to time too.

As Lucent said, "If sequence infringement is deliberate by a certain member, we will hand out warnings accordingly."
Isn't there a way to simplify it? Like an additional Custom User Title or something? To simplify things and leave that thread for special cases.

Sounds quite complicated and unnecessary to me. I have faith that moderators will not abuse their powers, and if they do we will let them know.

Why is there an age restriction on this?

Because: The Children's Online Privacy Protection Act of 1998 (COPPA) has to do with the cost and work to comply with the law. And as told by some users, children under the age of 13 are annoying/immature at handling mature material.
I'm going to get a lot of *****it for this but make chat +15 (15 or more) only. You can use the site normally but not use or view the chat with 13 or 14, maybe send or receive PMs even but that may be an overkill.
